ADMISSION INFORMATION

    Target Population: Girls aged 12-18 are referred to Brooke Place from a variety of sources. Referral sources include, but are not limited to

  • WVDHHR
  • Juvenile Probation
  • Regional Multi-disciplinary teams of community providers
  • Community mental health agencies
  • Therapists
  • Attorneys
  • Families

    Referrals can be started over the telephone but need to be backed up with written information regarding the client and her family. This information typically includes:

  • Biopsychosocial summary
  • Psychological evaluations
  • Psychiatric treatment/medications
  • Discharge summaries from previous placements/hospitalizations
  • Family involvement/motivation for treatment information
  • Medical needs/problems
  • Educational status

    A pre-intake interview is arranged for the client and her family. During the interview/assessment, questions are posed of the client and family members to help us determine what the needs are and if Brooke Place can assist in meeting those needs. There are different outcomes of the interview. Outcomes may include:

  • Occasionally it is decided that Brooke Place is too restrictive of an alternative, that home-based services would better serve the needs of the client and her family.
  • Other times it is decided that Level III residential care is needed.
  • Sometimes it is believed that drug/alcohol rehabilitation is more appropriate before the client attempts a setting like Brooke Place.
  • Our ability to provide education in the public school setting must be taken into account. Some clients require residential facilities that have on-grounds schools.
  • There must also be a clear commitment from the family to participate in family therapy if the plan is for everyone to work toward reunification. Brooke Place accepts girls who say they want to work toward transitional living, but even in these cases, it is important to have some level of cooperative family involvement.

    If it is decided that placement at the group home is appropriate, contacts are made with the interested parties-the caseworker, probation, other agency, family, etc. A date is set for intake.

    The group home has had success with clients fitting these general guidelines:

  • Client is in the temporary custody of the Department of Health and Human Resources.
  • Client has had specific, significant behavior expectations set for her by her parents and/or by the Court.
  • Client is able to identify some of the concerns that have led to the referral being made.
  • Client has a family who will be involved in the treatment process.
  • Client is able to express herself verbally.
  • Client's full scale IQ on the WAIS or WISC-R is 85 or higher.
  • Client's involvement with drugs/alcohol is largely limited to experimentation or mild to moderate usage.
  • Client's safety and safety of other residents can be maintained.

    Brooke Place does not maintain a waiting list for residency. Referrals are considered in conjunction with openings available and on a high risk-high need basis. If you are a caseworker and wish to view our current openings,
